Title: The Pre-Operative Assessment and Management of the Medically-Complex Patient

Topics: Pre-operative assessment and management of patients with common and complex medical co-morbidities undergoing dermatological surgery, including: immunosuppression, bleeding tendencies, impaired mental capacity, pregnancy, cardiac patients and those with complex medical and social needs.

Audiences: For all grades of Dermatologist as well as other Health Care Professionals with an interest in Dermatology

Learning Objectives: At the end of this module, the learner will have a clear understanding and knowledge
of:

  • The pre-operative assessment of patients with common and complex medical co-morbidities
  • Deciding appropriate management strategies based on patient and surgical factors
  • How to take a holistic approach to patient care
  • How to optimise surgical outcomes
  • The assessment and management of skin cancers

Synopsis:
This innovative and interactive module is built around a series of real-life clinical cases, covering both common and more challenging patient scenarios. The resource covers assessment of immunosuppressed patients, including infection risk, management of high risk NMSC and consideration of cosmetic and functional impact of facial surgery pre-operatively. Drug safety, and skin surgery in pregnant patients are also considered, in addition to risk factors for poor scarring, and management of keloid and hypertrophic scars. The module covers assessment and management of patients with impaired mental capacity and frailty, including decision-making strategies to optimise outcomes. Managing cardiac patients with a cardiac device, including anticoagulation considerations and indications for antibiotic prophylaxis. Assessing and managing Haematology patients, particularly those with an increased bleeding. The module also explores the surgical reconstructive ladder; factors to consider when choosing a surgical repair; as well as the importance of taking a holistic approach, particularly in complex patients with multiple comorbidities.

Learning objectives:

To understand different skin biopsy techniques (punch, curettage and incisional) and the indications for these.
To improve practical skills at performing simple surgical biopsy procedures.
To review indications and practical tips for performing cryotherapy.

Course description:

This course is based around separate modules on punch biopsy, curettage and cautery, incisional biopsy and cryotherapy. Each module is designed around learning through user interaction.

This module covers important aspects of Consent & Medicolegal Matters including the Montgomery ruling, how to achieve informed consent, managing complaints and how to avoid and deal with wrong site surgery

At the end of this module, the learner will have a clear understanding and knowledge of:

  • How to best achieve informed Consent and the 6 common pitfalls in the consent process
  • How to manage patient complaints
  • How to manage the threat of being sued
  • How to avoid and manage wrong site skin surgery

Synopsis

This innovative and interactive module uses videos and interactive features and questions to cover the topics of informed consent and the Montgomery ruling as well as the common pitfalls in the consent process. It also covers how to best manage complaints and the threat of being sued and wrong site skin surgery- how and why it can happen and how best to avoid it. It takes approximately 60 minutes to complete and allows the learner to dip in and out of its sections.

This innovative, interactive module covers giving local anaesthetic painlessly and safely and how to manage potential toxicity; how local anaesthetics work and the different types; and local anaesthetic use in pregnancy.

  • How to give local anaesthetic safely and manage local anaesthetic toxicity
  • How local anaesthetics works and the different types of local anaesthetics
  • Local anaesthetic use in pregnancy

 Synopsis

This innovative and interactive module uses videos, animation and interactive features and questions to give practical tips and advice on local anaesthetic use. It takes approximately 60 minutes to complete allowing the learner to dip in and out. Clinical cases are used to cover how to give local anaesthetic optimally and as painlessly as possible; how to give local anaesthetic safely and manage local anaesthetic toxicity; how local anaesthetics works and the different types of local anaesthetic; and local anaesthetic use in pregnancy.

This innovative and interactive module covers important anatomy in the head and neck; danger zones on the face, how to manage bleeding, nerve blocks, and surgery in the periorbital area.

  • Topics: Head and Neck anatomy, danger zones on the face, facial blood supply, managing bleeding, nerve blocks, periorbital surgery.
  • Audiences: All grades of Dermatologist

Learning Objectives

At the end of this module, the learner will have a clear understanding and knowledge of:

  • Important anatomy in the head and neck region
  • The path of the facial nerve and its danger zones on the face
  • Erb’s point in the neck and its importance
  • Facial nerve blocks
  • The blood supply of the face and how to manage bleeding optimally
  • Undertaking surgery in the periorbital area and potential complications

Synopsis

This innovative and interactive module uses videos and interactive features and questions to cover important anatomy in the head and neck, including the path of the facial nerve and its danger zones on the face, and Erb’s point. It also covers the blood supply to the face; how to manage bleeding optimally; and when and how to tie off vessels etc. Cadaveric dissections detail important sensory and motor nerves on the face and how to undertake nerve blocks; as well as undertaking surgery in the periorbital area.
