Event enquiry form test

This form will appear on all event pages. It automatically reads the event title uses this to populate the form and the email that is sent to the user.

All you need to confirm is:

1) The content of the form (i.e., which fields do you want)

2) The content of the email that is sent to the user (i.e., what nice message do you want to send to the delegate)

3) As well as sending an email to the delegate, it can send a copy to you. Do you want this sent to your personal email address or to some other address?

I can then set this up and put the form on all event pages.
